Revocation of "Opt-In" to Overdraft Fees for ATM and Everyday Debit Card Transactions

If you would like to revoke your current "opt-in" election, please complete and submit the following online form.


I am revoking my "opt-in" election. I no longer want Community State Bank to authorize and pay overdrafts on my ATM and everyday debit card transactions.
