You may limit the Bank and its affiliate, CSB Insurance Group, from marketing their products or services to you based on your personal information that they receive from the Bank. This information may include your application information, such as your income, your credit history or payment history.
Your choice to limit marketing offers from CSB Insurance Group will apply as long as you have an account relationship with the Bank.

To limit marketing offers, please notify us in writing within 30 days of this notice, and include the following information: Your name(s), address, social security number(s), and account number(s).
